Rehabilitation Coordinator - Muskoka
JOB SUMMARY:
Leisureworld Caregiving Centre – Muskoka is home to 182 residents and currently has an opening for a Permanent Full-Time Rehabilitation Coordinator. The initial assignment is for 75 hours bi-weekly, subject to change based on the operational needs or under the discretion of the manager.
As a valued member of the team, the Rehabilitation Coordinator demonstrates a commitment to resident safety by providing quality care in accordance with organization’s vision and mission.
RESPONSIBILITIES (But not limited to):

Manage and coordinate the planning, implementation, and evaluation of a wide range of Restorative Programs, including but not limited to Dining Program, Walking Program, Wheelchair Mobility Program, ADLs, ROM, Strengthening, Fine Motor, 1:1, small group fitness, based on resident needs.

Manage and provide training and education to Restorative Care Assistants.

Oversee and/or assist in completing assessments of Residents and the establishment of resident goals and interventions based on assessed resident needs.

Oversee and/or assist in the completion of documentation related to Restorative Care, including but not limited to assessment, daily flow sheets, progress notes, discharge, etc.

Participate in the homes Continuous Quality Improvement Program including but not limited to completing audits and ongoing program evaluation.

Complete care plans and evaluations for all residents participating in Restorative Care.

Develop and implement presentation on best practices in Restorative Care as needed.

Ensure effective communication with residents, family members, staff, and volunteers.

Oversee duties and processes related to Physiotherapy, Occupational Therapy, and Therapy Supply and Service Providers including but not limited to seating, assistive devices, and mobility aides.

Work collaboratively as part of a multidisciplinary team, including the resident, family, nursing, dietary, environmental, activation, PT, OT, and vendors to ensure interventions to promote resident independence are implemented.

Chair the Restorative Care Committee.

Participate in Multidisciplinary Care Conferences and corporate conferences as required.

Maintain awareness of changes within the profession and seek continuing educational opportunities.

Other duties as assigned.
QUALIFICATIONS:
-
Experience working in a Restorative Care Setting.
-
Must have experience in program planning, documentation, and evaluation.

Sound working knowledge of and understanding of the elderly.

Must demonstrate initiative and the ability to work independently and or in a team setting.
-
Must have well developed communication, leadership, time management and organizational skills.

Working knowledge of MDS RAI Documentation an asset.

Competency in electronic documentation systems, Outlook and MS Office an asset.
-
Must have effective verbal and written English communication skills.
EDUCATION:
-
Current registration as Registered Practical Nurse with the College of Nurses of Ontario with a Certificate in Nursing Rehabilitation from a recognized educational institution.
OR
-
Current registration as Registered Practical Nurse with the College of Nurses of Ontario with demonstrated knowledge of Restorative care and assessment and evaluation.
